It’s hard to win when you don’t create or know the rules of the game. By understanding the rules and learning techniques to succeed, physicians can improve and streamline clinical documentation and free their time for what they enjoy and do best, treating patients. E&M codes account for 50 to 85 percent of all billed codes and represent substantive revenue streams to a medical practice. The documentation that supports these billed codes is vital. Rather than existing as two disconnected coding systems, the single point of commonality between E&M coding and DRG coding, documentation of multiple medical diagnoses, bridges the gap between E&M and DRG codes, and correspondingly between physician and hospital.
